Health2040: Action Plan for Sustainable Outpatient Primary Care in Switzerland

 

The Health2040 initiative, a collaboration between the Swiss Learning Health System at the University of Lucerne and the Institute Interface Politics Studies Research, has published an action plan for sustainable outpatient primary care in Switzerland.

More than 50 stakeholders from healthcare, social sectors, and civil society contributed their expertise through a co-creation process. Among the participants were patients, service providers, representatives from federal, cantonal, and municipal levels, health insurers, social workers, and neighborhood support organizations.

The action plan outlines a comprehensive care network, the “Gesundheitsnetzwerk für alle (Health Network for all) – pour tous – per tutti”, designed to meet the needs of a changing society and to ensure long-term quality in healthcare. It includes three key areas of action:

  • Network-Based Care: The network promotes close collaboration between healthcare, social sectors, and civil society, fostering flexible, person-centered healthcare.
  • Coordination and Navigation: Effective coordination ensures that all services within the health network are aligned and that patients are guided through the system. A shared data platform supports efficient health promotion, prevention, and care.
  • Low-Threshold and Flexible Access: A low-threshold and flexible access system ensures that the health network is accessible to everyone. A multilingual app and locally anchored community spaces provide people with direct support within their living environment.

The organizations and individuals involved in the co-creation process are actively supporting the practical implementation of the action plan. The “Health Network for All (für alle) – pour tous – per tutti” serves as a shared vision of sustainable outpatient primary care for all stakeholders.
